The document presents CareFirst’s integrated approach to obtaining high quality, cost effective care services for CareFirst members, as well as for Medicare beneficiaries through the application of a “Common Model” (CareFirst’s PCMH Program) that applies one set of rules, a common infrastructure, one set of data and one set of accountability rules. CareFirst’s PCMH Program launched in 2011 and the Medicare effort got underway in 2012 under the auspices of a CMS Center for Medicare & Medicaid Innovation Award.
The Program Description and Guidelines is comprised of three volumes and eight distinct parts detailing CareFirst’s perspective on the problems and challenges that shaped our approach to the PCMH Program, as well as on each main program component.
