- EFT Payments Issued as Checks:CareFirst identified on or around May 10 an issue in which some providers who were set up to receive ACH/EFT payments instead received paper checks. This issue remains under active investigation. Providers do not need to take any action at this time. We are working toward resolution and will continue to share updates as more information becomes available, including expected timing.
- ERAs Reflecting Check Information Instead of ACH:CareFirst is aware of an issue in which some Electronic Remittance Advices (ERAs) reflected check payment information instead of ACH/EFT details. This issue was resolved on May 29, 2026. CareFirst is currently working to reissue impacted ERAs to include the correct ACH/EFT information. Providers do not need to take any action at this time.

The CAQH Provider Data Portal Application is an online application that is compliant with the Maryland and the District of Columbia Uniform Credentialing Form regulations.

- You do not have to fill out a new form every three years – just update any inaccurate information in your account.
Get Started
Follow these steps to use CAQH Provider Data Portal Application:
- Complete the CAQH Dental Provider Data Sheet and a W9.
- Email them to dentalcontracting@carefirst.com.
- Receive a CAQH ID number that allows access to the online application.
- Complete your online application and authorize CareFirst to access your information.
- Fax your supporting documents and attestation to CAQH at (866) 293-0414.
Once you receive the notification from CAQH that your application is complete, please email CareFirst at dentalcontracting@carefirst.com so that CareFirst can begin the credentialing process.
Already using CAQH?
If you have already completed an application through CAQH:
- Verify your data by logging in to your CAQH account.
- Authorize CareFirst to access your information.
- Complete the CAQH Dental Provider Data Sheet and a W9.
- Email them to dentalcontracting@carefirst.com.
