The Medicare Health Outcomes Survey (HOS), administered by the Centers for Medicare & Medicaid Services (CMS), is a patient-reported survey designed to evaluate Medicare members' physical and mental health outcomes over a two-year period. The survey is fielded annually from July through November. Provider support helps ensure patients complete the survey, which reflects
the quality of care provided and can influence CMS Star Ratings.
