New Form Helps Streamline the Process

In order to create a more efficient process for patients needing post-acute care, CareFirst has developed a new form to be used by inpatient facilities.

Facilities should fax post-acute care requests, using the completed form, to the dedicated Transition of Care team who will move forward with next steps.

Faxing the completed form allows the Transition of Care Coordinator (TCC) to:

• verify patient benefits,
• review clinical details,
• provide necessary approval or denial, and
• create the authorization.

Avoid the back and forth for post-acute facility services. Use the new form today.
