Md. Uniform Treatment Plan (UTP) Form Now Includes Applied Behavior Analysis (ABA)

The revised version of the Maryland UTP form, which now includes a field for ABA services, is now available online. To download a copy of the form, visit www.carefirst.com/providerforms > Behavioral Health.

As a reminder, CareFirst treats ABA as a medical benefit.

Reminder: Md. Online Pre-Auth Requirement

Effective July 1, 2015, all Maryland providers will be required to submit all medical and pharmacy pre-authorization requests online.
