Important Update: Simplifying Utilization Management Fax Submissions
As shared in our What’s New for 2026 update, we are implementing improvements to our Utilization Management fax intake process. This began with the reduction to one dedicated fax line: 443-753-2346 for Medicare Advantage requests effective February 1, 2026.
As we continue to streamline operations and enhance efficiency, our next step will be transitioning from our current system of 30+ authorization request fax forms to just two forms to handle all request types. This change is designed to support a new technology solution that will simplify and expedite fax submissions.
Important: Electronic submission through the CareFirst Provider Portal remains the most efficient way to submit prior authorization requests. Fax submissions should be used only in rare situations when the provider portal is not available.
What’s Changing
- Fewer Fax Forms: Instead of multiple forms for various authorization request types, providers will now use only two dedicated forms. One for Inpatient requests and one for Outpatient requests. These new forms are available to access on the provider website and will cover all prior authorization requests for all lines of business.
- The new forms can be accessed now on the provider website Forms pages:
- Consolidated Fax Numbers:The number of fax lines has also been reduced. Each line of business has a single, unique fax number for all requests. This will eliminate confusion and ensure more timely processing of requests.
