Submit NPI on All Paper Claims - CMS 1500 and UB-04

April 15, 2011 - Effective July 1, 2011, CareFirst will require that all paper claims submitted (CMS 1500 and UB-04) include your National Provider Identifier (NPI) in the appropriate fields. Though prior to this the NPI number was optional, this new requirement ensures improved efficiency of claims processing.

Where do I put my NPI number on the CMS 1500?

Enter your NPI number in:

  • The non-shaded area of Box 24J
  • Box 33a

Note: You should continue to enter your CareFirst 8-digit rendering ID number in the shaded area of box 24J. Additionally, the BlueShield ID Qualifier (1B) and CareFirst legacy ID number can continue to be entered in box 33b.

Where do I put my NPI number on the UB-04?

Enter your NPI number in field FL56.

Note: Your legacy ID can continue to be entered in FL57a. There are no other changes in the requirements for NPI information in other FL fields.

Apply through the National Plan and Provider Enumeration System (NPPES) developed by the Centers for Medicare & Medicaid Services (CMS).
