CareFirst Renames Transition of Care to Continuity of Care

CareFirst BlueCross BlueShield and CareFirst BlueChoice, Inc. (CareFirst) has renamed Transition of Care to Continuity of Care to differentiate this process from the Hospital Transition of Care Program.

Your patients who receive care from an out-of-network physician for an unstable or serious medical condition may be eligible for the Continuity of Care process. New members can receive care from an out-of-network provider for up to 90 days following the date of enrollment and pay at the in-network level.

Eligible conditions include:

  • Bone fractures
  • Pregnancy
  • Newly diagnosed cancer
  • Recent heart attack
  • Joint replacement
  • Other acute trauma or surgery
  • Other serious medical conditions where the member is in active treatment
