New Somatus Care Management Program for Patients with CKD/ESRD

Pilot Begins in Washington, D.C. and Northern Virginia

Beginning January 1, 2021, Somatus*, a leading kidney care management company, is offering an integrated care management program for CareFirst BlueCross BlueShield (CareFirst) patients with chronic kidney disease (CKD) and end-stage renal disease (ESRD).

This program is being piloted with 250 members in our commercial, fully insured groups with policies issued with Washington, D.C. and Northern Virginia. Pilot participants must also reside in one of these areas. These services are offered to eligible CareFirst members through their benefits at no additional cost. Groups written in Maryland are currently not participating in the pilot.

Patients in the program will be connected with a Somatus care team including a nurse care manager, community health worker, renal pharmacist, renal dietitian and social worker. This care team will support the patient in the following areas:

  • Nutrition and health coaching
  • Medication management
  • Behavioral health
  • Closing gaps in care
  • Social services
  • Transplant options
  • Advanced directives
  • Modality education and access management

In the coming weeks, Somatus will send physicians in the pilot areas an introduction letter and FAQs outlining the program.
