Palliative Care Support for Medicare Advantage Members

CareFirst BlueCross BlueShield Medicare Advantage, along with Aspire Health, offers a palliative care program to members at home who are facing advanced illness.

The program is a non-hospice, community-based alternative focused on:

  • Providing care aligned with the patient’s goals, values, and choices
  • Helping ensure patients understand their diagnosis
  • Facilitating conversations with patients and their families around the patient's goals of care

Palliative care is provided by a care team of doctors, nurse practitioners, nurses, and social workers who visit the member in their home. Physicians and nurses are available to patients 24 hours a day, 7 days a week. Aspire does not replace the care of primary care providers (PCPs) and specialists. This care team supplements the member’s current care received from their providers, and members may continue to seek curative treatment. The program is provided at no cost to members.

To help keep a member’s current PCP informed, providers receive an introductory letter when their patient is enrolled in the program and is regularly contacted by the program’s care team to discuss any significant changes to care plans or medications.
