How HealthyBlue Works

HealthyBlue members will be asked to complete an online Health Assessment and then schedule a PCP visit to conduct a Health and Wellness Evaluation at no out-of-pocket expense.

When your patients come in for their appointment, ask them to bring:

Through the Health and Wellness Evaluation, the member's baseline health status for certain risk factors will be determined. These health factors are determined by the American Journal of Preventive Medicine to be most indicative of future health outcomes.

Health factors include:

  • Tobacco Use - required for ages 18 and older
  • Blood Pressure - required for ages 18 and older
  • Cholesterol (LDL) - required for ages 18 and older
  • Body Mass Index (BMI) - required for ages 2 and older
  • Influenza Immunization - required for ages 2 and older
  • Childhood Immunization Status - as appropriate for age
  • Common Cancer Screenings:
    • Breast (50+ women)
    • Cervical (21+ women)
    • Colon (50+ men and women)

Measure and record your patients’ results for each health factor on the Health and Wellness Evaluation Form.

Remind your patients to submit the completed form within 180 days of their effective date to be eligible to receive their reward.
