What is an HTC?

  • HTCs are Hospital Transition of Care nurses who cover hospital facilities in Maryland, the District of Columbia, and Northern Virginia.

What is the role of the HTC?

  • After a hospital admission, HTCs triage CareFirst patients to determine which services they could benefit from upon discharge.

The Post-Acute Placement Process - A Refresher

Hospital Transition of Care Nurses

  • Collaborate with the facility discharge planner for all requests for post-acute placement.
  • Send an email to the Post-Acute Transitions of Care Team at CareFirst to request post-acute placement. The email includes the facility contact information for both the requesting and receiving facilities.

The Post-Acute Transitions of Care Team

  • Verify the members benefit for the requested service.
  • Give the requesting facility discharge planner a verbal approval for transfer.
  • Create the authorization and notify the receiving facility of the authorization number and number of days approved.
  • If there is no benefit or the clinical information does not meet medical necessity for the requested service, the team will notify the facility requestor and discuss alternative options.

Avoid the back and forth for post-acute, inpatient facility services.
