How it Works
Members can earn rewards for completing healthy activities within a certain timeframe, depending upon their effective date. One main step is to schedule a visit with you to get a health screening. The health screening is designed to help our members gain an understanding of their current health status and risk factors. Members should bring a health screening form with them to their appointment with you.
When our members call your office to set up their appointment:
- Work with them to schedule an appointment so they have time to submit their results to CareFirst.
- Ask them to get lab work completed prior to their appointment (if applicable) so you can review their results with them.
- Remind them to bring a copy of the Health Screening Form and any immunization/screening information that your office may not have on file.
At the office visit, you will measure and record your patients’ results for certain health factors on the health screening form, including:
- Weight/Body Mass Index (BMI)
- Flu Vaccine
- Tobacco Use
- Blood Pressure
- Blood Glucose
Starting January 1, 2023, members can download the health screening form by logging in to or creating a CareFirst WellBeing account at carefirst.com/wellbeing, or by calling 877-260-3253. Once available, they will need to enter their screening results in their account by navigating to Blue Rewards Program, clicking Complete a Health Screening, then Enter Your Results.
