What is kidney failure? 
Kidney failure happens when damage results in loss of normal kidney function. There are 2 types of kidney failure—acute and chronic. 
Acute kidney failure starts abruptly. It can sometimes be reversed. 
Chronic kidney failure progresses slowly over at least 3 months. It can lead to permanent kidney failure. This is also called end stage renal disease (ESRD).

PCMH & CKD

All Medical Care Managers (CMs) will identify and assess Members with abnormal kidney function and/or high-risk indicators for CKD onset using the CKD smart filter in iCentric. CMs will review all identified Members with their assigned PCPs and work together to develop Care Plans, when appropriate.

CMs should follow the steps below: 

  • Filter the CKD list for each Provider.
  • Target Members with an IBS between 5 and 8 and focuses his/her attention on the lists as follows:
    • Priority Focus
      • Class 0: Needs Screening – give the list to the provider/office staff for follow-up
      • Class 1: Low Risk - No CKD or other indicators of kidney disease; determine need for repeat eGFR/ACR
      • Class 2 & 3: Evaluate for Care Coordination and Member outreach
    • Secondary Focus - no further action required by CMs in Northern VA and DC. These members will be picked up by our vendor, Somatus. Service expansion to Maryland is anticipated 2022.
      • Class 4 & 5: Evaluate status of Member connection with kidney services and compliance with treatment regimen. Discuss with PCP when there are gaps to determine best approach.
  • Integrate CKD lists into monthly Clinical Status Review (CSR) for Members on the CKD list (IBS 5-8) with the Provider
    • Review identified individuals with the PCP and utilize the CKD Treatment Recommendations (revised 2021) document to assist in deciding an appropriate course of treatment, the need for a Care Plan, the frequency of kidney function monitoring, and the timing of referral to kidney care specialists and other related community-based resources (e.g., renal nutritionists).
  • Upon assessment with the Provider, assign an Assessment Outcome (AO) and CKD Disposition.
