High-Risk Pregnancy Program

The High-Risk Obstetric (OB) care management program provides coordination of care to your patients with high-risk pregnancies. Dedicated nurse care managers collaborate directly with each member, OBGYN, Maternal Fetal Medicine (MFM) and any ancillary support services required to ensure members’ needs are met.

Our care managers meet with your patients one-on-one by phone to answer their questions, make sure they understand your recommendations and connect them to needed resources like nutritionists, social workers and in-home IV hydration or drug therapy. They can even set up transportation to prenatal appointments.

While early identification and intervention is preferable, our care managers work with at-risk expectant women at all stages of pregnancy and can help your patients with:

  • Gestational diabetes
  • Gestational hypertension/preeclampsia/postpartum preeclampsia
  • A multiple pregnancy
  • Advanced maternal age
  • Chronic or comorbid health conditions
  • Preterm Labor/previas
  • Kidney Stones/Gallstones/Trauma that occurs while pregnant

Our nurse care managers also provide assistance after the baby is born such as lactation support, help setting appointments and connecting members to resources for post-partum depression.
