Want to be a part of CareFirst’s innovative Patient-Centered Medical Home (PCMH) program? Enroll today.

The following information must be completed on behalf of each practice for enrollment in the CareFirst PCMH program. All Primary Care Physician(s) and Certified Nurse Practitioner(s) within the practice must agree to participate in the PCMH program.

Each of the practices in a Virtual Medical Care Panel must fill out and submit Attachments A, B, and Addenda together in one packet before the application can be processed.
